    The parameter separator in the URL is being interpreted as an entity
reference open; an XML parser sees '&subaction' and says where is the
entity reference close (';').

    You need to escape the '&'s using '&amp;'s as follows (line wrapped by me):

subaction=html&amp;fileName=client-inquiry.html'>
