Log Message:
Fixed bug in Namespace.getNamespace() where the lookup for a 
pre-existing identical namespaces would fail even if there was 
an pre-existing identical namespace.  This caused new Namespaces 
to be created on all Namespace.getNamespace() calls!!!

Thanks to Thomas Koch <Thomas.Koch at atlantec-es.com> for first mentioning
this on jdom-interest a while back and putting me on the trail of this bug.

Also removed old Javadoc remark about how a prefix could be switched on 
you, because we're not doing that anymore.
